Call for awards nominations!

 
CSHP-NL Annual Awards – Nomination Deadline September 15th!  
It is that time of year when nominations for our branch awards are being accepted to be given at our SAM and Fall CE day. The deadline to nominate a colleague or mentor is Sept. 15th. Nomination forms, along with the supporting documentation, should be sent directly to Stephanie Young, Awards Committee Chair (E-mail: swyoung@mun.ca). We strongly encourage you to nominate your fellow members for well-deserved recognition!

Alfred G. Dawe Award of Excellence This award will be given to a pharmacy professional who is member of CSHP NL Branch and has shown significant and extensive contributions to the activities of the Branch. This award does not have to be awarded on an annual basis. Selection Criteria: The candidate must have provided sustained service to the CSHP NL Branch and have been a long-standing member of CSHP (typically 10 or more years). Nominations shall be requested from the CSHP NL Branch membership and must be made by two active members of the Branch. Nominations shall be submitted to the Awards Committee in writing no later than September 15 of each calendar year. The nomination form must describe clearly the individual’s contribution to the CSHP, with particular focus on Branch activities. Examples of such activities would include involvement with various committees, task forces, or councils of CSHP. The sharing of information on contributions made to or on behalf of the Branch not covered specifically through committee work is also encouraged. For example, participation in job shadowing programs, educational programs to members, etc.

Leadership in Pharmacy Practice Award This award is intended to recognize significant contributions to the practice of pharmacy within Newfoundland and Labrador. Recipients of this award will have demonstrated this contribution through leadership in areas such as clinical practice (excellence in the provision of patient-focused care), academia, advocacy or pharmacy management, and administration. This award will be given to a pharmacy professional who is a member of CSHP NL Branch and, through their dedication to the practice of pharmacy, has achieved in advancing the profession and enhancing (either directly or indirectly) the level of care provided to patients. This award does not have to be awarded on an annual basis. Selection Criteria: The candidate must be a member of CSHP NL Branch with long-term contribution to the practice of pharmacy. Nominations shall be requested from CSHP NL Branch membership and must be made by two active members of the Branch. Nominations shall be submitted to the Awards Committee in writing no later than September 15 of each calendar year. The nomination form must describe clearly the individual’s contribution to the practice of pharmacy as outlined above. Preference will be given to those candidates with sustained CSHP membership.

New Practitioner Award The New Practitioner Award is intended to recognize a new pharmacy practitioner who exhibits promising leadership, dedication and commitment to practice excellence and professional growth. This award will be given to a member of CSHP NL Branch who, through their service to patient care, to education or research, to the profession and to the Canadian Society of Hospital Pharmacists, is deemed worthy of recognition. This award does not have to be awarded on an annual basis. Selection Criteria: Completed their first pharmacy program within the last 5 years. Has been a member of CSHP for a minimum of 1 year. Has shown dedication and commitment to their practice and the profession. Has made an exceptional early contribution to pharmacy practice. For example, patient care initiatives; education of healthcare practitioners or the public; publications or presentations; research; participation in CSHP activities. Preference will be given to those who have shown some contribution to CSHP. Each candidate must be endorsed by at least two CSHP NL Branch members who can provide independent validation of the candidate’s contributions to pharmacy practice and contribution to CSHP.

Mentorship Award The Mentorship award is intended to recognize pharmacy professionals who have a positive influence on students, residents, and peers. This award will be given to a pharmacy professional who is a member of CSHP NL Branch and who, through their dedication to teaching and mentoring, have made significant contributions to the practice of pharmacy. Selection Criteria: Must be a member of CSHP NL Branch Should have at least three years’ experience in: mentoring and precepting pharmacy learners (e.g. student pharmacists or student pharmacy technicians, pharmacist or pharmacy technician interns, pharmacy residents, registered pharmacy technicians, or registered pharmacists) participating in the training of fellow pharmacy professionals in areas such as (but not limited to) patient care activities, pharmacy research or pharmacy administration Exemplifies effective teaching and mentoring skills which motivate and encourage students, residents, and peers Creates opportunities for learners to gain valuable knowledge and skills A letter accompanying the nomination form must describe clearly how the individual meets the selection criteria as outlined above Preference will be given to those candidates with sustained CSHP membership. The nomination must be submitted by two individuals: CSHP NL Branch member or supporter, and A mentee
